Michael, is there a time frame in which we need to submit a review? My expectant dcm isn't due until the end of Sept. so I could give it to her or use it myself. To be honest, I'm thinking using it myself would be the best option because who knows when she'd get back with a review. (just thinking out loud here)

I am trying to figure out where we do the review? It sent a sheet of questions but does not say where to mail it back to or where to go online to give the review.

I will ask on a time limit but I would think Sept would be too far away.
