It's been a whiles since I have been on. Hope all is well.

Would any one happen to have a waiting list form that they could share.

I don't have a form for my waitlist but I have a spot on my website where parents can contact me in regards to their care needs and their contact info and then I put them on my waitlist, which is really just a folder in my e-mail that I just save their emails in.

Every month I send out an email to the families on the waitlist letting them know of any changes in enrollment or possible upcoming openings and they must respond to the email within 3 business days. If they don't respond, I remove their name from my list and carry on.

So no real form just a contact form on a website that gets e-mailed to me. Hope that makes sense.
